Personalized care for Medi-Cal members
Enhanced Care Management (ECM) is a free Medi-Cal benefit that helps people with complex health and social needs get the care they deserve. Through ECM, you can get support with medical care, mental health, housing, and other services, all coordinated by a team that works with you.

The goal of ECM is to bring together all the services you need to improve your health and reduce gaps in care. We focus on:

If you qualify for ECM, you will have your own care team, including a lead care manager. This person will coordinate with your health care providers, pharmacists, schools, social service providers, and other support staff, to ensure everyone is working together to meet your needs.

The Enhanced Care Management (ECM) — Birth Equity Program is a Medi-Cal benefit that targets racial and ethnic health disparities and ensures pregnant and postpartum individuals get the care they need for a healthy pregnancy and beyond. We provide personalized support to improve access to high-quality care and create a better birthing experience.
Each eligible member is matched with a dedicated care team, including a Lead Care Manager (LCM), who works closely with you, your healthcare providers, and community partners.

If you're enrolled in a Medi-Cal managed care plan, you may qualify for Enhanced Care Management. We support both adults and children who need extra help getting connected to care and services.
Populations we support include:
Didi Hirsch is a contracted ECM provider for the following managed care plans: LA Care, Molina, and Healthnet.
