We value your feedback and want to make sure your voice is heard if you have concerns regarding your experience with us. Consumers, their legal guardians, family members and personal representatives, have the right to file a complaint or grievance with Didi Hirsch related to the care, treatment or services you received from Didi Hirsch.
What you can report
You may express a complaint related to concerns or dissatisfaction with services received, the Didi Hirsch staff, or the environment where you received services. We encourage you to first discuss your complaint(s) directly with your service provider or program supervisor to seek a resolution. Please bring these concerns up as soon as they happen, so we can address them promptly.
Filing a grievance is a more formal way to report serious concerns or problems that you feel have not been resolved by the care team or are not appropriate to address with your care providers. This may include concerns that your rights were violated, your confidential and private information was breached, or concerns regarding the quality or safety of your care.
How to submit a complaint or grievance
We encourage you to share your complaints directly with individual program staff, supervisors, or program leadership.

Contacting the Joint Commission about quality or safety
If you have concerns about the safety or quality of care you or a loved one received, you have the right to be heard. In addition to reaching out to us directly, you may also share your feedback with the Joint Commission—an independent organization that works to improve health care for everyone.

Your rights are protected
You will not be subject to discrimination, retaliation, or any other penalty for filing a complaint or grievance. You may ask staff for assistance with completing the grievance form, if you have an accessibility need.
